Role Overview
As an Advanced Practitioner, you will be responsible for assessing, mentoring, and overseeing the care of clients with various support needs. You will play a crucial role in ensuring the highest standards of care through assessments, staff mentoring, and service improvements.
Key Responsibilities:
- Conduct initial client assessments and develop tailor-made individualised care plans
- Observe, mentor, and assess clinical staff performance
- Oversee session and care plans, providing suggestions for improvements
- Develop and manage an online Activity Resource Library
- Provide clinical guidance and escalate concerns as needed
- Be available to assist staff in emergency or critical situations
- Ensure client records are updated and follow-ups are completed on time
- Provide clinical insight to support service development
Requirements:
- Qualifications: Degree in Psychology, Nursing, or an Allied Health Profession
- Experience: Previous experience working with vulnerable groups
Skills and Knowledge:
- Familiarity with a range of disabilities, including dementia and autism
- Ability to work within an intra-disciplinary team
- Strong problem-solving and crisis-resolution skills
- Excellent communication and mentoring abilities
- Organised, creative, and proactive approach to care
